It means that a computer at IP address 204.160.111.116 tried to connect to you, 76.98.129.**
The "sequence number" is basically a counter of all the pieces of data sent from that machine to yours.

Basically, your router said, "Whoa. I wasn't expecting that packet. Just to be safe, I'm going to ignore it."

By chance, are you having problems with particular websites?
