Title: Does a factory reset require reformat?

Just curious:  Does performing a factory reset necessitate a drive reformatting?  i.e. does resetting it to the factory settings erase the partitions and data on the drives, or does it just reset the parameters?

Also, if I can do it without reformatting, will I also need to re-map the drives, or will the networked computers automatically connect if I just reset the usernames/passwords/groups?

No, I've done it a number of times on my DNS-323 and DNS-321.

Did you need to reset all your permissions as well, or do those get maintained?

You'll have to configure all the accounts again, it truly sets it back to factory defaults. :)  The data is retained.
