Hey, I am stuck in port forwarding for Synology DS211+ problem.
My network configuration is Internet Modem (from ISP model: ZTE ZXDSL 931CIIA) DGL-4500(version 1.15) local PC(wired), DS211+ NAS(wired),
PS3(wired), Iphone(wireless), Ipad(wireless) and IBM notebook(wireless).
I read around and found out some information from there and other forum. I created a static IP(192.168.0.199) for the local PC, static IP (192.168.0.198) for NAS and also enable DHCP reservations on DGL-4500 ensure that the all devices keep the same IP address.

When ever I try to open specific ports and then test the ports, both the www.canyouseeme.org and www.yougetsignal.com show a timed out error and says it's blocked. I already tried a bunch of different ports and no avail. Anybody have any suggestions?

What ISP Service do you have? Cable or DSL?
What ISP Modem do you have? Stand Alone or built in router?
Ensure DNS IP addresses are being filled in under Setup/Internet/Manual?
Turn off Advanced DNS Services if you have this option.
Turn on DNS Relay under Setup/Networking.
Setup DHCP reserved IP addresses for all devices on the router.
Ensure devices are set to auto obtain an IP address.
Set Firewall settings to Endpoint Independent for TCP and UDP.
Turn off all anti virus and firewall programs on PC while testing.
Turn off all devices accept for one wired PC while testing.

I am not sure, I think the service is DSL due to I need to input username & password.
I think the modem is built in router.
Sorry to tell you that, I don't understand what is your meaning of DNS IP addresses. Whish IP address require to input.
My setting is as shown below:

You mean NAS is need set to auto obtain an IP address

Is the DSL modem in bridged mode? I presume it might be?
Primary and 2ndary DNS IP addresses should be filled in, you can find these value under Status/Device Info/WAN section.
If you have set up reserved IP addresses for all of your devices on the router then, the NAS aswell should be set to auto obtain and IP address from the router.
Enable SPI since your using Endpoint Independent.

In the synology webinterface, ther is an option for port forwarding to your router. Look if your router is supported, en use these settings. It worked for me. Look also on www.synology.com, en enter into the forum.
Don't give your nas an dhcp adres. But reserve an adres, en enter the adres into your nas.

If the DSL modem can be put into bridge mode or already in bridge mode, then port forwarding on the Syology modem wouldn't work probably.
It is recommended to use reserved (static) IP addresses for all of your devices, including NAS.

If it's 192.168 then the DSL modem isn't bridged.
Bridging mean that if a DSL modem is a built in router itself, it effectively turns it off, by passing any routing processing and handing off an actual public IP address through the modem to the 4500 router which is preferred.
Most DSL modems have bridging capabilities. Call up your ISP and ask them if they can help you. I'll probably need your PPPoE DSL account user name and PW information to input into the 4500 router under Basic/Internet/Manual/PPPoE connection.

In the router Status/Device Info/WAN section IP address is 42.98.XX.XXX which will be changed after the modem was reboot.

Ok so this indicates that the modem is in bridged mode and handing off a public IP address. Yes, it's ok if this IP address changes and it will.
Make sure the Primary and 2ndary DNS addresses you see under the WAN section is copied and paste these into Basic/Internet/Manual if these are not filled in already.

These are addresses coming from the router and are correct for the device(s) that you are using.

May I have an other question.
Regarding the remote management the router outside my local network,
I have already enable the remote management on router and the DDNS is enable and connected. But I cannot access via http://xxxxxxxx.dlinkddns.com:8080 to remote my router.
Could you please give some advise to find out what is problem?

Thats one thing I haven't done yet is Remote Mgmt using the router settings.
I mainly use TeamViewer.
Maybe Hard Harry can be of help here. ::)
