Click the link for the phpMyAdmin web interface and configure it.
Username / password default: admin / admin

Did you reboot the NAS after installing - don't ask me why but some addons only function correctly after reboot

I have the same problem with the username and password, default is not working neither the restart. Anyone can help ? Thanks in advance.

Anyone noticed that when you add the newer version of phpMyAdmin v4.0.10.10 that there is a message to install updates? If you install the updates teh software is regressed back to v3.5.1.0?
