Call quality issues with VOIP

Howdy everyone,

I've been battling call quality issues for a while with my D-Link RP system.  We started with POTS lines, then got into the NGT VOIP program.  We experienced static and noise when using POTS, and now we're dealing with gaps, garbled sound, and static noise.  We've been working with NGT for the past several month in trying to iron out this issue.  We've tweaked our QoS Edgemarc, we've talked to our ISP in checking for dropped packets, and we've investigated our own internal network (which is Gb). 

We're at a point where our ISP is telling us all is fine (and I get zero dropped packets when doing extensive ping tests to various IP addresses), and NGT is telling us that their carrier, Level 3, isn't seeing any issues with our calls.  I'm beginning to suspect the PBX itself.

We have four DPH-125MS phones and one Aastra phone.  Our PBX is D-Link's.  The system has been upgraded to SP2, with all phones upgraded to the latest firmware that was just recently released.  Our Edgemarc is the network gateway, everything Internet related funnels through it.  Our ISP supplies us 7Mb download, 1.5Mb upload.

Any help would be appreciated.

What are you using for network switches ?
Make, model?
Have you setup QoS on every port and device?

I'm having some issues also, using pots lines, mostly with jitter and garbled voice.  I'm begining to suspect our brand new DES-1228 may be the issue, as we've RMA'd our entire response point system because of an echo, and still have the same problem with the new system. I've also spent a few phone calls with support and making sure the qos/tos is setup correctly.

I have a DES-3010fa 8 port layer2 managed switch that i'm planning on isolating the phones and RP equipment to see if that gets rid of the echo or makes any improvement.
