If by factory reset you mean tools>system>restore factory defaults, I did try that as I mentioned on my post.
I've tried firefox, chrome, internet explorer and of course Safari on Itouch. Yes, same thing unfortunately.
As for the ISP modem, it is a Motorolla Surfboard Cable Modem (SB6120)

I believe that is a stand alone modem? I'm not sure, you could probably tell me better.
I have had that modem operating with my dir-655 for about a year and a half now, I've virtually had zero problem ever since I set everything up. Even right now, everything works flawlessly except for me being unable to access my university affiliated sites. Which is kind of a big issue considering I need to access it fairly regularly to keep up to date on my academics.
Advanced dns service is turned off (unchecked).
DNS Relay is turned on (checked).
Most of my devices seem to already have reserved ip addresses under setup/networking. I'm only not seeing the ps3 on there, surely that's not what's causing this issue? I'll add it to the list regardless.
Do you mean the obtain ip address that can be found at Control Panel\Network and Internet\Network Connections > right click > Internet protocol version 4/and/or/6? If so they're all checked on obtain it automatically.
I'm not sure how to set the firewall to what you suggested, but I've tried turning it off completely, no luck. And like I've said, I can access the sites no problem if use my modem directly to connect to internet, even if firewalls/anti-virus protection are turned on. It's just when the dir 655 is used this problem rises.

^And yes I did try chrome's suggestion too to no avail.
