As I understand it, your having a problem with Media Sharing content from a computer to the PS3? A, any traffic to or from your DSL modem/router is irreverent. Both your PC and PS3 are in the same Local Area Network(LAN), so NAT doesn't come into play. NAT is involved when you send data to a connection somewhere out in the interwebz(as in not local), but when that site responds, it needs to know what computer on your network made the request. All the site can normally see is the public IP of your modem, and thus your router. Since this is all local content, neither device is contacting the outside inviroment. Furry's advise was good, but I think he thought you meant streaming media from PSN or some other place, and not movies on your computer.
As for your log, most of that is normal entries. If I had to guess, your modem's private IP(gateway) is 192.168.1.198, and your PC was opening up port 52569 as part of a security scan done on Speedtest.net? Like this

I would guess 192.168.0.3 is your PS3, because the blocked TCP packet was heading to one of the servers of PS3's content delivery provider(think of them as Fedex for Internet Video). By the way, watch out what info you post on this public forum because that IP also tells me there is a good chance you live in Arizona.
So yea, long story short; Modem is fine. Streaming should work even with modem unplugged. Not sure what the problem is, need more info.
1. Are either the PS3 or PC wireless?
2. What else happened a month ago? Any new software on the PC? Firmware update on the PS3? Moved either? Anything at all, never know whats important.
3. What kind of content are you streaming? We talking HD movies or youtube quality stuff? Movies or movies and music?
4. Is there ever a time since last month that it worked great?
5. How many PC's do you have in your network?
