Did you check to make sure the app is enabled? Also, I've found that the surveillance app (at least on the DNS-345), only works on IE.

How I can check Enable or not? I see icon in index page, click not opening. Do not use IE, only Safari, checked in Chrome.

Right-click on the My Surveillance app icon and you should have the option to either Enable or Disable the app. Alternatively, you can open the web UI and navigate to Management > Application Management > Add Ons.
