Just want to say first I have already tried Xbox Support all the troubleshooting on their end didn't work. ::)
My Xbox live connection drops all the time pretty much whenever it wants it is random most of the time. It is on a wired connection, I have done all the port forwarding, tried DMZ. 95% of time I can log back in right away.
I have dsl modem, it was bridged before and I have called to verify that it was. I have my 645 thru the dmz with reserved IP right now -it was the last thing I tried- My laptop works internet works fine, it is connected wireless. Also if it connected directly to the modem it works just fine.
Firmware is 1.01 and used the guide that was here to do so.

What Hardware version is your router? Look at sticker under router.
What region are you located?
Are you wired or wireless connected to the router?
Check the XBL sticky for XBL in this forum.
What ISP Modem do you have? Stand Alone or built in router?
What ISP Modem make and model do you have?
If this modem has a built in router, it's best to bridge the modem. Having 2 routers on the same line can cause connection problems.
To tell if the modem is bridged or not, look at the routers web page, Status/Device Info/Wan Section, if there is a 192.168.0.# address in the WAN IP address field, then the modem is not bridged.
Some things to try:
Turn off Advanced DNS Services if you have this option under Setup/Internet/Manual.
Turn on DNS Relay under Setup/Networking.
Setup DHCP reserved IP addresses for all devices ON the router. Setup/Networking
Ensure devices are set to auto obtain an IP address.
Set Firewall settings to Endpoint Independent for TCP and UDP under Advanced/Firewall.
Enable uPnP and Multi-cast Streaming under Advanced/Networking.
What wireless modes are you using? Under Setup/Wireless/Manual.
Try single mode G or mixed G and N?
Channel Width set for Auto 20/40Mhz or try 20Mhz only.
What security mode are you using? Preferred security is WPA-Personal. WPA2/AES Only. Some WiFi adapters don't support AES, so you might want to try TPIK only or Auto.
What wireless devices do you have connected?
Any cordless house phones?
Any other WiFi routers in the area? Use InSSIDer (http://www.metageek.net/) to find out.
Turn off Short GI and Extra Wireless Protection if you have it. Under Advanced/Advanced Wireless.
Turn off all anti virus and firewall programs on PC while testing. 3rd party firewalls are not generally needed when using routers as they are effective on blocking malicious inbound traffic.
Turn off all devices accept for one wired PC while testing.
Check cable between Modem and Router, swap out to be sure. Cat6 is recommended.

I called to verify that was modem was bridged, since viewing the ADSL page once it bridged isn't the easiest thing, well for me at least.
Advanced Dns - wasn't there
Dns Relay - is on
Reserved all IP addresses and are set to auto obtain
Firewall is set to Endpoint
uPnP and Multi-cast is Enabled
Set to mixed G and N
channel width 20 Mhz
WPA personal, set to auto
2 smartphones, Printer, Kindle Fire are all the wireless devices
no cordless phones
One Network that is on a different channel then I am
Short GI is off
Problem is with my Xbox so no antivirus
All Cables are working and cat 6
I will post again after I test Xbox connection
Edit: after testing the connection, it failed twice to connect, and connected the 3rd. Then disconnected while I was on the dashboard.

Tried in DMZ and bridge neither kept their connection nor could I connect game servers thru Xbox live.

Once the DSL modem is bridged, you will loose it's web page access. Thats normal.
Check the QoS gaming sticky for XBL (http://forums.dlink.com/index.php?topic=44869.0) in this forum.
Maybe someone can review your router settings with you using teamviewer (http://www.teamviewer.com) if your interested. Its safe and secure.

So after fighting with it for the past hour :-\
I did all the QoS Settings, Now the router won't get the IP address from the bridged modem -_-
I directly connected the bridged modem to my laptop and it work. Also tried it with my Xbox and live worked great, which didn't make me any happier really.
So what steps do I take to fix all this because quite honestly as good as wireless is for this model, the trouble it has given isn't worth it.

have you powered OFF both Modem and router then power ON the modem first and let it sync then power on the router and check to see if it gets an IP address?

What IP address did you see on the Status/Device Info/Wan Section? 192.168 or a public IP address from your ISP modem?
Lets do a factory reset on the 645 and reset up the router with out QoS.
Possible you might update to Beta FW.
Let me know if you want some one to review the router settings.

Ok did the factory reset, I get the public IP from my ISP

Then go in and set up QoS.

I am doing it manually, I do have a config saved and I made a new one after I finished doing it this time.

I reserved the PC and the Xbox I took everything else off the network for now.

Before the QoS i couldn't even log into a game server but it logged me instantly, The NAT was Open. Odd how QoS did what DMZ couldn't ??? Something is wrong with this picture! :(
Well I test some more and get on some servers so see if the connection fails or not. I am hoping for the best, I will report back in a day or so with an update.

Just to update furry looked at my settings, changed a few. Was working great last night.
Today on the other hand no so good I stayed connected longer but I still get dced, the only up side is that it connected quick now. Just have to find out how to keep it connected lol.
Any more idea's? Or would it be best just to make it an AP since my ISP's modem can do routing?

Was this playing MW3 or just connecting to XBL dash board?

Was playing ME3 at the time, got kicked off the game server, then couldn't log back in for a while. Only got kicked off XBL once after that.
Connection to XBL is speedy and stays connected longer as far as I can see but I am still being dced for some odd reason.

Ok, I'm going to see if any one at Dlink can look at this.
Possible could ideas I have:
1. This router could have a compatibility issue with XBL as I don't see it on MS XBL HW compatibility list.

2. This router could have and issue itself and needs to be RMA'd. Couple of ways to test this out, Take this router to a friend or families place with internet and xbl or take your xbox with it and see if the problem tracks with it. I feel that since you had been gaming with the ISP modem before that the problem will still track, However thats not to say that there is a possible compatibility issue between the ISP modem and the 645 as well. Why it could be a good test to try a different ISP modem if you can find that option. One last option would be to go to a local store that has a refund policy and get a different DLink router or even try a non DLink brand and test to see if a different router exhibits the same thing.
3. If this ISP modem only provides Internet, I might ask the ISP for a different DSL modem, even if this is the latest and test. Any chance they have a modem only with out a router?

One thing I have noticed is after the QoS changes the speed matches the ISP modem directly. Where as if I tried to download anything before it would take me about a day, now about an hour. So with out the changes I was definitively being throttled by the 645.

I have asked my resources about this. Will see what we get back.
Let us know if you can try a different router or take this one to a different location.
BTW, I have the DIR-657 and it works well on XBL, at least on my cable ISP. ;D

I was googling for the same situation and found this post.
I have a D3 cable modem connecting to 645, same issue, cannot get connection to Xbox live nor PSN (Playstation Network),
I've opened the TCP/UDP ports as exactly the same as my previous router, DGL-4500,
but there is no luck with DIR-645 so far with multiple tries....

Need to remove the Port Forwarding and use QoS on the 645 and use Specific rules and not weighted. use a open rule and enter the IP address of the xbox and select P2P from the drop down menu for the preset protocols. Save and reboot the router. Ensure you have reserved the IP address for the xbox as well.

I don't use PPPoE since my ISP doesn't require it, I going to pick up a cheapy router and try it out, it will be good for me to have a back up any way

Ok, sounds like this is a possible issue with the 645. I'll be online later and or this weekend if you want to do some more testing. Let us know about netflix. How long have you had the 645 for?

Over 2 months ordered it 18 Jan 2012.
Well do about that netflix, sometime this weekend would be fine.
Update: Tested Netflix and it works great.
While the 2.4 Ghz signal isn't as good as the 645 it works for now.

Seems like there could be an issue with the 645.
I would recommend doing an RMA maybe.

I got my 645 yesterday and installed it. Pretty nice I might say.
I didn't turn ON QoS or set up any thing for XBL. I played for about 2 hours last night on MW3. Had a good time. Saw a few things with LAG, felt that was MW3 and others. Router did very nicely. I'm going to try with QoS enabled and see out it goes tonite.
