DIR-625 and QoS for 2 vonage devices

I have 2 vonage devices (home and work) on my network with IPs in my private subnet, both behind a DIR-625 router. The QoS rules will only let you use Port numbers once for a given IP address. So how can one establish QoS for second vonage unit for same Port numbers? Vonage recommends ports 10000-20000 or 25000.
Having major problems with Vonage devices and Cox. Everything was working flawlessly for 5 years with 1 vonage device, and 2.5 years with 2 vonage devices until about 4 months ago and problems did NOT follow any changes to my network config internally (connections or config in router).
We did switch to static IP addresses from Cox with higher speed business connections. That is when problem started primarily with home vonage unit - essentially when my path went thru different Cox servers. I placed the problem unit outside my router but problem persits. Cox is supposedly monitoring spikes and noise issues.
