Well unless you stated the model wrong, one site reports the 602G being a Gateway:

Thus the reason why a direct connection would work if there is NAT on the Modem.

Just sayin...

How ever you stated "The cable modem does have a status page 192.168.100.1 and also provides an IP address to the router that is 216.xxx.xxx.xxx so there is some type of NAT happening."

If this is the case then configuration of the 655 needs to be effected properly.

I had a 602g before I upgraded my service.  The 192.168.100.1 page was informational only, there was no configuration that could be changed.

I presume that some ISP probably either bridge the modems or also lock out features that they might not want users to see as well.

Well, seems like this could just be a configuration of the 655. IF he's getting forwarding while directly connected to the ISP modem and isn't with the 655, some configuration is needed for his application.

Reset to factory settings.

Opened up the port in virtual server section.

Working now.
