There should be 3 MAC addresses, WAN, LAN and WiFi. LAN and WiFi should be the same I believe.
Did you factory reset, upload FW then factory reset once again then set up from scratch using IE or FF?
Where did you get this 4500 from?
I'm curious why your not getting speeds as others are. The only thing I can think of is that there could be an incompatibility between the ISP modem and the 4500. Any chance you have a neighbor, friend or family member near by with a different modem model that you could test the 4500 out on? Kind of need to verify the 4500 with a different ISP modem if possible before ruling out the 4500 could be bad.

You could attempt this and see if speeds are any different:
Turning a router into an AP.Unbridge the ISP modem and see what the speeds are while connected to the 4500 in this mode...
