HELP!! Cannot install MySurveillance add-on on DNS-340L

Urgently needs help!

For some reasons My Surveillance add-on stopped working  :'( i tried to reinstall the add-on and it is when nightmare starts...

i could not get My Surveillance installed regardless how I tried... i tried installing 2.05 from the GUI without success, says "Install Failed"

manual install the add-on downloaded from following locations without luck - error "Install failed. Please check file."

my DNS-340L runs on firmware 1.02 (shipped by default)
i tried downgrading to 1.01 with no luck, fw downloaded and tried:
